Insurance (Medical/Dental/Vision/Long Term & Short Term Disability/ Supplemental Life Insurance): Full time employees are eligible for coverage the first of the month following 90 days of continuous employment. To be considered a full time employee, you must average 40 hours of work per week. In the event of an approved Family Medical Leave of Absence, employees must pre-pay premiums in order to continue coverage. All insurance premiums are deducted on a pre-tax basis, from every paycheck, through payroll deduction. Upon termination of employment, coverage ends on the last day of the month in which the employee terminates.
Life Insurance: Full time employees are eligible for life insurance coverage of $15,000 at company expense, effective the first of the month following 90 days of continuous employment. Additional supplemental coverage is available at employee expense.
Paid Time Off: Both full time and part time employees earn paid time off based on hours worked and completed service with the company. Employees do not become eligible for vacation until the first of the month following 90 days of continuous employment. All paid time off is paid based on the employee’s hourly base pay rate. All requests for vacation must be submitted to the supervisor a minimum of one week in advance on an Absence Request Form.
Holidays: Full time employees are eligible for holiday pay, based on their hourly base pay rate.
